6 month member agreement

1. Membership Duration

• The membership term is six (6) months, beginning on the date of purchase.

• Member is billed $120, monthly

2. Member Responsibilities

• Members must comply with all The Fit Asylum rules, policies, and staff instructions.

• Proper workout attire and footwear are required at all times.

• All equipment must be cleaned immediately after use.

• A signed liability waiver is required prior to participation.

3. Facility Rules

• Members must check in at the front desk upon arrival.

• Lockers for use at the member’s own risk. The Fit Asylum is not responsible for lost or stolen items.

4. Health & Safety

• By signing this agreement, the member affirms that they are physically able to engage in exercise activities.

• Members must inform staff of any medical conditions that may affect participation.

• The Fit Asylum is not liable for injuries sustained while using the facility or participating in classes.

5. Membership Pause Policy

• Members may request to pause their membership for up to two (2) months within the six-month contract term.

• Pause requests are permitted for medical reasons only and must include:

  • A written request from the member, and

  • A valid doctor’s note.

• All requests are subject to management approval.

• Approved pauses will extend the membership end date by the length of the pause.

6. Payment & Cancellation Policy

• Membership fees are billed automatically each month and are non-refundable.

• Early cancellation is not permitted.

• All payment obligations must be fulfilled for the duration of the contract.

7. Suspension or Termination

• Membership may be suspended or terminated without refund for:

  • Misuse of The Fit Asylum property,

  • Misconduct or unsafe behavior, or

  • Violations of these terms and conditions.
